For Book 1 include:

• Which Book you want to sign (ie Inside JW, Apprentice,Residential, VDV, Apprentice VDV,  VDV Trainee, CW, CE,CWR, and/or StockPerson) 
• Your termination slip
• Your Name
• Address including city, state and zip code
• Phone #
• E-mail

For all other Books include:

• Your termination slip or a Letter of Introduction from your local stating you are currently unemployed.
• Name
• Phone #
• SS#- Min of the last 4 digits. (If you do not want to email your SS#: Leave a voice mail with your name, phone number, and SS#, on the Jobline, (608) 255-0169. Speak slowly and clearly.)
• Birth Date
• Sex
• Ethnicity- optional but recommended for reports to the state.
• Address including city, state and zip code
• IBEW Card #
• Local #
• Classification (JIW, Apprentice, VDV etc.)
• Which Book you want to sign (i.e. Inside JW, Apprentice,Residential, VDV, Apprenticee VDV  VDV Trainee,CW, CE, CWR, and/or Stock Person)
• Any other qualifications (i.e. fire alarm, OSHA 10/30, CPR, Med Volt Splicing, Welding, etc)
• WI Electrical license # and expiration date
